Benefits Eligibility

Benefits are effective on your hire date. You are generally eligible for CGW benefits if you meet the below requirements.

  • You're regularly scheduled to work at least 30 hours per week

  • You're on the US payroll

  • You're not a temporary employee, agency employee, or consultant

 

Dependents' Eligibility​

  • Spouse: A spouse is the individual to whom you're lawfully married but not legally separate

  • Domestic Partner: A domestic partner is an individual of the same sex or opposite sex with whom you're in a domestic partnership, civil union, or other similar formal relationship that's not a marriage under state law.

Children Eligibility (up to age 26)​

  • Your biological children

  • Your stepchildren

  • Your domestic partner's children

  • Your legally adopted children, including children placed with you for adoption

  • Children for whom you're responsible as a legal guardian or under the terms of a QMCSO

Domestic Partner Eligibility

For a domestic partnership, other than one that is legally registered and valid, in order for the subscriber to include their domestic partner as a dependent, the subscriber and domestic partner must meet the following requirements:

  1. Both persons have a common residence.

  2. Neither person is married to someone else nor a member of another domestic partnership with someone else that has not been terminated, dissolved, or adjudged a nullity.

  3. The two persons are not related by blood in a way that would prevent them from being married to each other in California, or if they reside in another state or commonwealth, that state or commonwealth.

  4. Both persons are at least 18 years of age.

  5. Both persons are capable of consenting to the domestic partnership.

  6. Both partners must provide the plan administrator with a signed, notarized, affidavit certifying they meet all of the requirements set forth in "1" through "5" above, inclusive.
